By entering my electronic signature below, I understand that MultiCare has partnered with BabyBeat to provide me with an audio Doppler and blood pressure monitor to use during my virtual visits. This kit also includes; ultrasound gel, batteries, instructions for use, and a return shipping label to be used to return the kit at the end of my pregnancy.

I understand that if I do not return the kit (BOTH the Doppler unit and blood pressure monitor) to BabyBeat at the end of my pregnancy, I will be financially responsible for the full purchase price of $279.99, which will be billed to the credit card entered.

All units are warrantied for coverage against damage, but not loss or theft. Unit must be returned to BabyBeat.
